How does Support Coordination work?
Support Coordination helps you to understand and implement the funded supports in your plan and links you to community, mainstream and other government services.
CDS SA Support Coordinators assist NDIS participants to understand and utilise their NDIS plans to reach their short-term and long-term goals. We work closely with participants, their informal, formal and mainstream supports to ensure participants NDIS plans are being coordinated effectively.
Our team of Support Coordinators work closely with the participant and their families/caregivers to ensure the participant is confident and building their independent skills to coordinate their supports, such as:
- Researching and building positive relationships within the community to establish appropriate community, mainstream and funded supports.
- Liaising with the NDIS and completing pre-planning reports and collating assessments prior to the participants planning / annual review meeting.
- Responding and dealing with crisis in a timely matter.
